Find out how Community Health Needs Assessments and community developers can build healthy communities at this year's Healthy Homes & Communities Summit!

Learn about what's happening between hospitals, community developers, public officials and other stakeholders across the state to improve the health of residents and neighborhoods. You'll also hear from funders and state officials on efforts underway to build healthy homes and communities and how you can get involved.
